Has anyone been approached to be a mentor?  Or anyone care to share what their firm does for a formal mentorship program?

I am at Raymond James and they have a formal mentoring program.  In this program, newbies are assigned a mentor.  The mentor makes themselves available for coaching.  As the newbie meets their goals, the mentor receives bonuses.

Like anything, it all depends on the people involved.  My relationship was very positive.

Who would approach an apprentice for mentorship? The janitor?
