Anyone get the claim form in the mail regarding OT lawsuit? California reps get $134.10 per month of eligible svc going back to Sept 1998. Tier 2 states, NJ, PA and NY get $90.12 per month  and all others appear to get $53.64 per month. CA goes back the farthest . NY to 8-30-99, NJ to 9-21-2000, PA to 5-5-2003. The others are between 8-2003 and 6-2005. Forms say these are the amounts after the lawyers get their cut.
