Does anyone have a sample of a resignation letter that an FA would submit to the firm he is leaving?

I know it should at least contain the following

Your letter should state you are leaving according to "protocol" and you are only taking your client's "birthday" info, you don't anticipate any "issues" and your clients can reach you at ABC Firm. Should also indicate you do not waive your right of arbitration should any problems arise and a name, address and phone number for your new firms legal counsel. 

Do you really give them the name addy and phone of the new firm? 

And I know I should do it Friday before the Monday I start.  Right?

I cant wait to jump this sinking ship

I'm pretty sure you are suppposed to say that you think  your manager is a "poopy head." Check with a lawyer first though.

it would be nice if I said he was a poopy head.

just learned I need to send a copy direct to my B/D too!  so to Branch Manager and B/D cause I dont want poopy head to hold it.  uggh help me.
